PocketCM and My Possibilities Announce New 3-Year IDD Care Coordination Partnership

A New Chapter in IDD Care Coordination Starts Now
PocketCM and My Possibilities — New 3-Year IDD Care Coordination Partnership

We're Partnering with My Possibilities to Bring IDD Care Coordination to North Texas

Today, PocketCM and My Possibilities are announcing a new 3-year partnership — and I want to tell you directly why this matters, what we're doing together, and why I think it signals something important for the IDD community.

My Possibilities is one of the most extraordinary organizations I've encountered in the IDD space. They've built a 20-acre Campus for Higher Learning in Plano, Texas that gives adults with Intellectual and Developmental Disabilities something genuinely rare: a real university-style education, real vocational training, and a real shot at an independent future. Their students are HIPsters — Hugely Important People — and My Possibilities treats that name like a promise in everything they do.

Starting now, PocketCM will support their team with purpose-built AI care coordination software. And I want to be honest about what that means in practice.

Who Is My Possibilities — and Why This Partnership Matters

My Possibilities was founded in 2008 by three mothers who looked at what was available for their children with IDD after high school graduation and found a gap. No full-time continuing education. No vocational training designed for their needs. No serious pathway to employment or independence in North Texas. So they built one from scratch.

Today MP operates a 20-acre campus in Plano — the first full-day, year-round IDD education program of its kind in North Texas. Over 450 HIPsters attend weekly, with programming covering life skills, vocational training, job placement, and socialization in a model that genuinely respects the dignity and potential of every person they serve.

What PocketCM Will Do for the MP Team

When I look at what My Possibilities' direct support professionals and care coordinators face day to day — the documentation load, the compliance requirements, the complexity of individualized support plans — I see exactly the problem PocketCM was built to solve.

Our platform will support their team with AI-powered shift documentation, care coordination workflows, and case management tools designed specifically for IDD and HCBS providers. Not general healthcare software adapted for disability services. Purpose-built — for the way this work actually happens in the field.

The goal is simple: give every DSP and care coordinator at My Possibilities back the time and mental bandwidth that administrative overhead currently consumes. So they can do what they do best.

"My Possibilities' team is doing extraordinary work. They deserve tools that fit — not tools they have to work around. That's what this partnership is about."

— Vahid Behzadi, CEO, PocketCM

What This Signals for the IDD Sector

My Possibilities' decision to partner with PocketCM is a signal that the sector is ready to demand better. IDD care is not a simplified version of hospital care. It requires different documentation, different workflows, different compliance frameworks, and a completely different relationship model between staff and the people they serve. Technology built for that reality — rather than approximating it — produces meaningfully better outcomes for teams and for HIPsters.

This partnership is a proof point we're proud to build in public.
